Who We Are:

Rehabilitation Team

Family members and other caregivers also play an important role in the rehab process. Our team will include you in education and training to ensure a comfortable transition to home.

What We Do:

An interdisciplinary approach is used to formulate a goal-oriented plan of care for each patient. Our focus is on assisting patients to achieve their highest level of independence as quickly as possible.

Our rehabilitation staff are available afternoons and weekends for evaluation of new patients. Upon admission, our patients will be provided with equipment and oriented to their new environment (an integral part of our falls prevention program.)

Based on their evaluation, treatment plans are individualized and designed to meet our patients' goals. From balance to home safety, our highly skilled clinicians are the key to our patients achieving their maximum functional ability.

Treatment may include the following:

  • Therapeutic exercise - individual and group settings
  • Strengthening through use of recumbent bikes, recumbent steppers, functional trainer (pulley system with weights) or hand weights
  • Balance activities
  • Gait training with assistive devices as needed (parallel bars, walkers, crutches, canes)
  • Wheelchair mobility training
  • Functional activities instruction - bed mobility, stair climbing, car transfers, dressing, bathing, and homemaking skills with use of assistive devices as needed
  • Pain management through use of heat/cold, electrical stimulation, phototherapy, positioning, joint and soft tissue mobilization and movement
  • Bracing/splinting/positioning for optimal comfort, safety and support
  • Energy Conservation education
  • Joint Protection education
  • Recommendations for home modifications or equipment needed for safe transition to home
  • Home environment evaluation
  • Education/hands-on training for families and caregivers

In addition, our skilled nursing staff is IV-certified, experienced in wound care, ostomy care, enteral therapy (feeding tubes) and post-surgical care.

We welcome family members to attend therapy sessions. Your support is an integral part of the recovery process.
