Quality Social Services. From Admission to Discharge

In addition to two full-time social workers, we're proud to say that our admissions coordinator is also a licensed social worker. One of her tasks is to coordinate your admission-or that of a loved one-through the social worker or discharge planner at the hospital.

For short-term rehab admissions, discharge planning begins in advance of a patient's admission and is done in collaboration with the patient and his or her family. One of our social workers will assist in arranging services necessary for the return home. Before discharge, a therapist may accompany the patient on a home visit and suggest alterations in the home, if necessary.

After discharge, the social worker will follow up by phone to monitor the patient's continued progress and adequacy of home services.
