Upon admission, a physical therapist completes a comprehensive evaluation and establishes an individualized physical therapy treatment plan. These plans typically include a focus on the following:

Upon discharge, additional assessments as needed:
- Both PT and OT will visit home or the destination of patient prior to discharge
- Recommendations for equipment needed at home
